1. Passport validity period is at least 6 months from the estimated date of arrival
2. Two recent photos, 3.5 x 4.5 cm in size, with a white background, using good quality photo paper and clearly proportioned facial features when zoomed in at 70%. The ears must be clearly visible in the photo.
3. A sponsorship letter in English from the applicant's employer. If there are other people/family members traveling with you, please include their names and status in the letter.

• The sponsorship letter must include: Passport Number, Department Rank, Monthly Salary
• Copy of SIUP (Owner) and in English and NPWP must be attached if the applicant is a General Manager, Director, Main Director or Commissioner
• If the applicant is a business owner and does not have an official letterhead, the applicant can type the letter in English with the company stamp and attach a photocopy of the NPWP and SIUP (Owner) in English.
• If the applicant is sponsored by a child, please attach a copy of the child's birth certificate. This will prove the applicant's relationship to the child. → In English.
• If the applicant is sponsored by his/her in-laws, please attach a copy of his/her child's marriage certificate and birth certificate to prove the relationship → In English.
• If the applicant is retired, the letter can be typed on paper in English and signed.

4. Photocopy of the applicant's bank statement for the last 3 months (clearly stating the name and account number, including the last transaction) minimum IDR 50 million/person

5. Copy of the last 3 months' pay slip

6. Copy of Family Card (KK) + Resident Identity Card (KTP)

7. Copy of Marriage Certificate (Marriage Certificate)

8. If the applicant's child is on the trip and is still actively attending school, then you can add:

• Photocopy of Student Card (Student ID Card)
• School Confirmation Letter (Original School Certificate) in English
• Copy of Birth Certificate (Birth Certificate) in English
• Print Reservation Ticket

9. Hotel confirmation during the trip in Europe, complete with details of the applicant's name as stated in the Hotel Confirmation

10. Travel insurance valid for the duration of your stay in Europe and providing coverage of USD 50,000 or EUR 30,000. Travel insurance is required to cover COVID-19.

11. If there is an invitation from France and it is verified by the local government, the applicant must include a photocopy of the passport, CNI, and invitation letter from the inviter.

12. The form is completed and signed by the applicant. For children who do not have an Identity Card (KTP), the form must be signed by both parents.
